“Community Residences is supporting me to be the best I can be. I am now living in my own apartment. Staff helps me manage my routine each day and helps me solve problems. I am doing all kinds of things that I never thought I would be doing. I love it.”
CR’s residential programs provide neighborhood homes with a shared living environment. Every home is located within reasonable access to shopping, recreational, social, cultural and religious activities, health care facilities, transportation, and vocational opportunities.

Skilled Nursing and Private Duty Nursing
CR offers medical supports by a Licensed Practical Nurse or Registered Nurse in Medicaid Waiver homes to prevent hospitalization or admission to a nursing facility or ICF-IID facility. Examples of skilled nursing care include intramuscular injections, insertion of Foley catheters, dressing changes and teaching diabetes care. Continuous medical supports are considered Private Duty Nursing and are determined on an individual basis.
